Low Vision Clinic Referral

If you are a medical provider, caseworker, or agency that needs to refer someone to the Center, please read the sections below and complete the referral form.

The Low Vision Clinic at the Alaska Center for the Blind and Visually Impaired supports Alaskans experiencing vision loss by helping them increase independence through education, rehabilitation training, and assistive technology. Clients work with low vision specialists and have the opportunity to explore strategies and tools that may help maximize usable vision, including magnification devices, glare reduction options, lighting strategies, adaptive equipment, and assistive technology such as handheld magnifiers and screen readers. The center also provides orientation, intake support, referrals, and connections to additional services and resources that may benefit each individual. Low Vision Clinics are held regularly in Anchorage, Fairbanks and the Mat-Su Valley, and doctor referrals are preferred for these locations.

What Happens After You Submit This Form

After your referral is submitted, ACBVI staff will review the information provided and contact you to discuss the next steps.

Once you are contacted, staff may schedule an orientation appointment to better understand your needs and how ACBVI services may be able to assist you. During this appointment, staff may review intake information, discuss your current vision needs, talk through available services and resources, and help identify possible goals for support or training.

Depending on your needs and eligibility, staff may also discuss options such as Low Vision Clinic, support resources, or other available training.
