Make a referral to our Low Vision Clinic
 
If you want to keep doing the things that matter most in your life. . .
. . . And eyeglasses or medical treatment still leave you with vision loss that interferes with your daily life or work, an appointment at the Center’s Low Vision clinic can make all the difference.
 
Here’s what you can expect at the Low Vision Clinic
There is no charge for an appointment at the Clinic. In fact, the Center provides up to $100 in recommended magnifiers and other devices, thanks to generous donors.
During the clinic you will:
  • Visit with an optometrist and the Center’s certified low vision therapist.
  • Discuss your visual needs and goals and find answers about your eye condition.
  • Try out a variety of magnifiers, telescopes, sun lenses, writing guides, and other devices for near and far viewing to discover what works for you.
  • Learn about new techniques and resources that can help you in your daily life or employment.
  • Get help accessing other resources of interest to you.
  • Receive free non-optical aids such as a large print calendar.
  • Receive a written report after the clinic. With your permission a copy will also be sent to your doctor.

 

To make an appointment

  1. Call 248-7770 for an appointment or use the online referral form above.
  2. Ask your eye doctor to send us your eye report from a visit within the last year.
  3. If you have Medicare or Medicaid, please bring your card with you to your appointment.
  4. If you currently use magnifiers, please bring those with you also.
  5. Family members or care providers are welcome to take part.
