For a Center-based low vision assessment, call 248-7770 to schedule an appointment or use the online referral form.
 
Ask your eye doctor to send us your eye report from a visit within the last year (this may be emailed as an attachement to info@AlaskaBVI.org or faxed to (907) 248-7517).
 
You will be called before your appointment at the Center to answer any quesitons you may have.
 
If you have Medicare or Medicaid, please bring your card with you to your appointment.
 
If you currently use magnifiers and/or any other types of devices to help you, please bring those with you also.
 
Family members and/or or care providers are welcome to take part in the process which may take up to 2 hours:
 
  • The Center's Optometrist will review your eye doctor's report with you and ask about any recent changes.     
  • Next the Center's Low Vision Specialist will talk with you about what you want to be able to do, answer any questions you may have about your eye condition. 
  • They give you an opportunity to try different magnifiers and other devices that may be helpful to you in achieving your visual goals. 
  • You may also be shown several non-optical items such as large print timers, talking watches, and magnifying mirrors available to make daily living tasks easier.
 
A report will be written and sent to you as well as your primary eye doctor - with your permission - summarizing the recommendations made, including referrals to other sources of support as well as the devices
found to be useful to you. Thanks to the Gottstein Family Foundaiton, up to $125 of those devices may be provided to you at no cost and sent directly to your home.
 
A follow up visit to your home may also be recommended to mark appliances and ensure general safety.
